Patient Forms
Upon entering our office, our receptionist will welcome you as a valued member of our health community. We will request that you complete our patient forms, either at our office or available to download in the New Patient Center. This paperwork provides us with your health history and information on your condition.
Consultation
Next, you will have a consultation with Dr. John Colwell or Dr. Scott Roberts to discuss your health-related problems, concerns, and potential treatment options. This initial visit is designed for the doctors to learn more about you, your condition, and expectations, to determine how we can meet your goals.
Examination
After your consultation, Dr. Colwell or Dr. Roberts will perform a complete chiropractic examination, testing your reflexes and flexibility. Other standard neurological, orthopedic, postural, and physical tests will be performed as well. However, nothing will be done in our office without your consent.
X-Ray Studies
Depending on your specific condition, we may take X-rays as well. X-rays help us develop the most effective treatment plan for you, and alert us of any serious spinal conditions. Seeing their X-rays gives patients the understanding of their problem, and the hard data they need to be fully involved in their treatment.
Report of Findings
Once the information is collected and examinations are performed, Dr. Colwell or Dr. Roberts will give you an initial report of all findings and answer any of your questions including:
- What can be done for my problem?
- How often do I need to come in?
- What will my treatment cost?
After reviewing your health history, goals, and examining your spine and X-rays, Dr. Colwell or Dr. Roberts will discuss recommendations and notify you if your condition requires care with other providers. We will provide the best possible treatment and wellness program for your needs.
Treatment
Should the doctor's findings indicate that chiropractic would be beneficial for your complaint, you can receive a spinal adjustment right away. Your initial visit may also include physical therapy, and/or soft tissue massage, depending on what you and your doctor decide is the best course of action.
Wellness Program
Because Dr.Colwell and Dr. Roberts are concerned about your total health, they will suggest a wellness program for home. If you are in pain when you first come into our office, the recommendations may include ice or heat application instructions, certain activities or positions to avoid, and at-home exercises and/or stretches. The purpose of our wellness program is for you to achieve good spinal alignment, maximize your energy with a healthy diet, establish a realistic exercise habit, and maintain a positive mental state. We want to see all of our patients to live better, not just longer!
